We are so excited to have your student registered for SRSM middle school mid-winter trip this year!

The following steps will help you and your student be prepared for their upcoming trip. Please be sure each step has been completed. 

STEP 1

Be sure you have paid your initial deposit or full amount. This gives your student a reserved spot in our group for our  trip. If you haven't done so, please be sure to also fill out the online registration form here.

STEP 2

We will need a SRSM Release Form filled out and notarized (if we don't already have one on file for your child). If you need us to notarize the form you must send a copy of your drivers license in order for us to notarize without you present.

STEP 3

Complete the Young Life Consent form. Please use this link to digitally complete the form or print the form. Be sure to hit submit. You can also print, scan and return it via email or return them to the SRC office or to one of the SRSM staff. (Let us know if you need us to print you a copy.)

STEP 4

We will need a copy (front and back) of your child's medical insurance card, you can scan or take a picture of it and return it via email or return them to the SRC office or to one of the SRSM staff. (Feel free to come to office for us to make a copy.)

STEP 5

Make all trip payments via PayPal on this page. ALL payments MUST be made and ALL forms filled out by February 1, 2021. NO EXCEPTIONS! Your child's spot may be forfeited and any paid money will not be refunded.
